What is a communication passport?

  • A document or booklet that includes information about how the young person would like people to communicate with them.
  • They guide clinicians in making reasonable adjustments to support communication, engagement and effective treatment.
  • Communication passports can be useful in times of transition or where you might be in contact with a number of different people across settings. For example, if you needed to see a new clinician for one of your appointments.

 

Things to think about when completing a communication passport

Is there anyone who the young person would like to contribute to the communication passport?

Who should the communication passport be shared with?

Where might the young person use their communication passport?

Agree a time to review the communication passport

The communication passport can change and grow over time
